Pelvic Pain Perplexity: A Case of Post-Zoster Sacral Plexopathy

Abstract
Varicella zoster virus lumbosacral plexopathy is a rare and under-recognized disease that can mimic common spondyloarthropathy presentations. We present a 26-year-old male with no significant past medical history who presented with chief complaints of pelvic numbness and tingling. Extensive diagnostic work-up, including Lumbar MRI, Pelvic MRI, EMG and laboratory tests, was negative, except for elevated varicella zoster titers. The patient was subsequently diagnosed with post-herpes zoster lumbosacral plexopathy. Here we discuss presentation, proposed mechanism, and management of varicella zoster virus lumbosacral plexopathy.
