The Bullae Challenge: Anesthetic Considerations in the Face of Vanishing Lungs

Abstract
Giant Bullous Emphysema poses significant anesthetic challenges due to compromised lung function and the risk of bullae rupture. In this case we discuss a 54-year-old male with a past medical history significant for chronic obstructive pulmonary disease and hypertension who presented with acute onset of chest pain and shortness of breath found to have an extensive pneumothorax due to a ruptured pleural bleb. There are limited reports in the literature regarding the anesthetic management for patients with giant bullous emphysema undergoing surgery. We will discuss the presentation, mechanism, and anesthetic considerations to aid in the understanding of this complex clinical condition
