Pulmonary Hypertension: A Challenging Extubation

Abstract
We present a case of a 63-year-old female with a history of multifactorial Pulmonary Hypertension, CREST syndrome, and aortic stenosis who presented for a total abdominal hysterectomy for leiomyosarcoma. The patient was unable to be extubated in the OR due to desaturations and was subsequently managed in the Surgical ICU for an extended period of time secondary to inability to extubate. The patient had recurrent hypoxia and continued pulmonary hypertension, eventually necessitating progression to tracheostomy. With this case, we will discuss pathophysiology and management of Pulmonary Hypertension.
