Severe Hypercarbia s/p Sedation in a Patient With Severe Pulmonary Hypertension

Abstract
54 yo F with a history of severe pulmonary hypertension with RVSP 90-95mmHg and OSA presented to the operating room for partial foot amputation under monitored anesthesia care. During the case, the patient required airway manipulation to aid in ventilation. In PACU, she became acutely hypotensive with altered mental status. The anesthesiologist placed arterial line and ABG was drawn, which showed pH 6.95 and PaCO2 92. Patient then presented to surgical ICU where she was intubated on arrival. We will discuss the physiology of pulmonary hypertension, treatment options, and anesthetic considerations.
