Sudden Onset of Severe Headache in a Parturient

Abstract
36 yo F s/p SVD with CSE 5 days prior presented to the EC with sudden onset of a non-positional, severe generalized headache. The OB anesthesiologist was contacted, who recommended neurology consultation and CT head due to the presentation being inconsistent with a post-dural puncture headache. Extensive workup including MRI/MRA was negative for severe etiology. Epidural blood patch was considered at this time, but after in depth multidisciplinary discussion, neurology recommended against the procedure despite the patient's persistent headache. We will discuss non-positional postpartum headaches including the diagnosis, workup, and whether a positional component is needed to diagnose PDPH.
