Breathing For Two: Anesthetic Management of Severe Subglottic Stenosis in Pregnancy

Abstract
We present a 30-year old female, G2P1 at 24 weeks gestation, with a past medical history significant for subglottic stenosis during pregnancy. She experienced dyspnea with exertion during her first pregnancy and required balloon dilation to correct > 55% laryngeal stenosis. Similar symptoms returned during her second pregnancy and she was scheduled for suspension microlaryngoscopy with laser-assisted removal, balloon dilation, and jet ventilation. This case presented unique challenges due to airway pathology, maternal-fetal physiology, and ventilation strategies to minimize risks to mother and fetus. This case will discuss anesthesia considerations for non-obstetric surgery, obstetric airway management and jet ventilation.
