Perioperative Management Pitfalls: Vasopressor Extravasation in Septic Shock

Abstract

We present a case of an 80-year-old male in septic shock requiring urgent stent placement for obstructive pyelonephritis. Preoperatively, he was hypotensive on low-dose norepinephrine through a 20-gauge peripheral IV. Following induction under general anesthesia and successful surgery, the patient developed arm edema, pallor, and coolness, raising concern for extravasation of vasoconstrictive agents. This case highlights the risks of administering vasoactive medications peripherally in vasoplegic states. Prompt recognition and monitoring were implemented in the postoperative setting. This case will discuss peri-operative management of septic shock, vigilance in IV site selection, and limb preservation strategies in septic patients requiring vasopressors.
