Description
Lyme disease is the most common vector-borne illness in the United States, with 1-4% of cases presenting with Lyme carditis. Of these cases, 90% of patients present with atrioventricular block (AVB). Ventricular arrhythmias are a described manifestation of disseminated disease, but are exceedingly rare, particularly without evidence of fulminant myocarditis.
A 19-year-old male from Northern Michigan presented after suffering a syncopal event. Upon EMS arrival, he was in sustained monomorphic ventricular tachycardia. He underwent an emergent DCCV resulting in sinus rhythm with complete heart block. He recalled spending time in wooded areas and noticed a "bruise" on his left shoulder 3 weeks ago.
Lyme carditis should be considered as a reversible etiology for ventricular arrhythmias, particularly in prevalent geographic regions presenting with concurrent AVB.
