Abstract

We present a case of a 19-year-old male, with a history of Burkitt leukemia, who developed sudden-onset severe diffuse polyarthralgia of unclear etiology, resulting in significant functional decline and withdrawal from college. Initial evaluation and oncologic workup were negative. The pain was refractory to multiple therapies, requiring the use of oxycodone, which has since been tapered off. Persistent symptoms include positional orthostatic tachycardia, sleep disruption, and disabling pain. Current evaluation includes POTS, Ehlers-Danlos syndrome, rheumatologic causes, and post-viral etiologies including EBV or COVID reactivation. With this case, we will discuss the comprehensive management of refractory diffuse pain in a medically complex patient.
