Abstract
A 39-year-old male with no significant past medical history presented with acute altered mental status and was initially found to be COVID-19 positive. Further evaluation revealed right-sided otitis media and mastoiditis on imaging. His clinical course rapidly progressed and was complicated by bacterial meningitis and intracranial extension of infection, resulting in a right-sided subdural empyema. He required urgent multidisciplinary intervention, including a craniotomy for empyema evacuation and a mastoidectomy. This discussion will review the progression of otogenic infections to intracranial complications, emphasizing early recognition and intervention to reduce morbidity and mortality.
