Laryngeal Cleft in a Neonate with VACTERL Association: Perioperative Management Considerations

Abstract

A 4-week-old male with a past medical history of VACTERL association- including persistent left SVC, ASD, VSD, and grade 2 hydronephrosis of the left kidney- presented with feeding intolerance. Further evaluation revealed a type II laryngeal cleft extending into the cricoid inferior to the vocal cords. He underwent endoscopic repair with CO₂ laser ablation of the cleft. Management required multidisciplinary coordination among neonatology, otolaryngology, and anesthesiology teams. This discussion will review laryngeal cleft classification, associated congenital anomalies, and perioperative management considerations.
