Refractory Hypoglycemia in an Emergent Exploratory Laparotomy

Abstract

We present a case of a 54-year-old patient with no significant past medical history who presented to the ED with altered mental status and abdominal pain. Workup revealed extensive colonic pneumatosis and severe hypoglycemia which initially improved with dextrose but continued to drastically decrease despite further treatment. The patient was taken emergently to the OR for exploratory laparotomy; repeat glucose levels intraoperatively and in the PACU continued to trend low despite further dextrose administration. With this case, we will discuss the differential of refractory hypoglycemia including medication induced as well as insulinoma related, along with diagnosis and management.
