Nausea and Vomiting After General Anesthesia: A Case of Unmet Expectations

Abstract
A 35-year-old female with a history of PONV underwent laparoscopic cholecystectomy. Despite triple prophylaxis with ondansetron, dexamethasone, and a scopolamine patch, she developed severe nausea and vomiting in the PACU requiring rescue antiemetics and observation. She expressed frustration and distress as she expected “complete prevention” of the aforementioned symptoms. Despite multimodal prophylaxis, PONV remains common, leading to dissatisfaction when expectations are unmet. Preoperative counseling is crucial to set realistic expectations, as inadequate education can create anxiety and mistrust. Transparent discussions about potential outcomes may improve patient experience, reduce distress, and enhance PACU workflow by mitigating the need for prolonged postoperative care.
