VV ECMO For Management of Septic Shock-Induced ARDS and Air Leak Syndrome

Abstract
We present a 56-year-old female with a history of smoking, vaping, and recent COVID infection who developed acute hypoxic respiratory failure in the ED. Workup revealed possible septic embolism with necrotizing MSSA pneumonia. The patient was intubated for acute hypoxic respiratory failure and treated for septic shock. Her hospital course was complicated by a bronchopleural fistula after chest tube placement leading to hypoxic cardiac arrest. VV-ECMO cannulation was implemented eventually leading to evaluation for lung transplant. This case will discuss management of septic shock leading to ARDS, air leak syndromes, and escalation to ECMO while considering lung transplantation.
