Perioperative Fluid Management in Central Arginine Vasopressin Deficiency

Abstract

We present a 66-year-old male with a medical history of craniopharyngioma who presented to the hospital s/p transphenoidal resection of cholesterol granuloma 2 weeks prior with altered mental status, hypernatremia, decreased urine osmolality, polyuria, and bradycardia. After being stabilized in the ED, he was diagnosed with central arginine vasopressin deficiency. Several days later, a CT head showed interventricular air due to leakage from his external ventricular drain, requiring surgery to repair the CSF leak. In this case, we will discuss the pathophysiology of central arginine vasopressin deficiency as well as the perioperative management of these patients, including fluid management and desmopressin.
