Perioperative Challenges of Abdominal Compartment Syndrome in Complex Hernia Repair

Abstract
A 58-year-old male presented to the Surgical ICU with worsening abdominal pain and dyspnea following Tenckhoff catheter placement. He subsequently developed hypotension with altered mental status. CT revealed a rectus sheath hematoma near the catheter, and elevated bladder pressures confirmed abdominal compartment syndrome. He was intubated for acute hypoxic respiratory failure and underwent an emergent laparotomy with hematoma evacuation and catheter removal. In this presentation, we will discuss the unique perioperative challenges of abdominal compartment syndrome, including airway management, hemodynamic instability, and coordination of emergent surgical decompression.
