Perioperative Management of Combined Liver-Kidney Transplantation Requiring Massive Transfusion

Abstract

We present the case of a 49-year-old male with end-stage liver disease secondary to alcohol abuse, hepatorenal syndrome, and chronic kidney disease, who underwent a combined liver-kidney transplant. The procedure was complicated by hemorrhage with an estimated blood loss of 30L, necessitating extensive resuscitation, including 66 units of packed red blood cells, 59 units of fresh frozen plasma, 14 units of platelets, 14 units of cryoprecipitate, 10L of cell saver blood, 15.5L of crystalloid, and 7L of albumin. We will discuss the perioperative monitoring and management strategies utilized during a liver-kidney transplant procedure requiring massive transfusion.
