Abstract

Context: Unusual cytoplasmic constituents, including melanin and hyaline globules, have been described in conventional (clear cell) renal cell carcinoma (RCC). We have noted Paneth cell-like eosinophilic granules in conventional (clear cell) RCC. The purpose of this study was to evaluate eosinophilic granules for their pathologic nature in clear-cell RCC. Design: Nine cases of conventional (clear cell) RCC were reviewed for the presence of Paneth cell-like eosinophilic granule and additional 22 clear-cell RCC were stained for CD68. Electron microscopy was performed to identify the granules. Results: All 9 clear-cell RCCs demonstrated eosinophilic granules, which were not present in chromophobe RCC (Figure 139). In total, 59% of clear-cell RCC were positive for CD68 in the cytoplasm, including the eosinophilic granules, which were found to be lysosomes by electron microscopy. Conclusions: Herein, we describe Paneth cell-like eosinophilic granules, a previously unrecorded, unusual cytoplasmic constituent in conventional (clear cell) RCC. Immunohistochemical and ultrastructural studies are supportive of their lysosomal primary nature in the cytoplasm of clear-cell RCC.
