Treatment Patterns and Visual Outcomes in Proliferative Macular Telangiectasia Type 2

Abstract
Purpose : Macular Telangiectasia (MacTel) type 2 is a bilateral neurodegenerative macular disease characterized by parafoveal capillary telangiectasias, retinal cavitation, and outer retinal atrophy. In its proliferative stage, subretinal neovascularization (CNV) can further threaten central vision. While anti-vascular endothelial growth factor (VEGF) therapy is widely used to manage CNV in MacTel type 2, real-world data on visual trajectories, treatment burden, and recurrence patterns remain limited. The purpose of this study was to examine real-world treatment patterns and outcomes of patients with CNV secondary to MacTel type 2. Understanding these parameters could help optimize surveillance and treatment strategies.
Methods : A retrospective chart review was conducted of clinical notes and optical coherence tomography (OCT) images from patients with proliferative MacTel type 2 who received anti-VEGF intravitreal injection. All patients had Snellen visual acuity testing and OCT at baseline and each subsequent follow-up. Patients were treated until CNV resolution. Descriptive statistics were used to summarize demographic and clinical characteristics of the cohort.
Results : A total of 22 eyes (17 patients) were analyzed. The mean age at CNV diagnosis was 63.8 years, and 58.8% of patients were female. The mean number of anti-VEGF injections was 2.8 until CNV resolution. 8 eyes received bevacizumab, 6 eyes received aflibercept, 5 eyes received ranibizumab, and 2 received combination therapy. The median BCVA was 20/40 before CNV diagnosis, 20/60 at diagnosis, and 20/50 at resolution. The mean central macular thickness (CMT) was 250.5 µm before CNV diagnosis, 271.1 µm at diagnosis, and 227.1 µm at resolution. Recurrence of CNV occurred in 7 of 22 eyes (31.8%) at a mean of 10 months following initial resolution.
