Scanning The Gap: Piloting A Structured Ultrasound Curriculum in OBGYN Residency.

Abstract

Introduction: While the American Institute of Ultrasound in Medicine (AIUM) has developed a formal ultrasound curriculum for OBGYN residency, implementation remains limited. Most U.S. programs lack structured training, and residents often report low confidence in ultrasound skills despite clinical exposure. To address this gap, we piloted a brief, structured session adapted from the AIUM PGY-1 curriculum as a first step toward a longitudinal educational program.

Methods: We conducted a prospective educational intervention with OBGYN residents at Corewell Health West Simulation Center (Grand Rapids, MI). Twenty residents (PGY 1-4) attended a one-hour session; eighteen completed both pre- and post-tests. The session included a didactic lecture on ultrasound physics and obstetric imaging, followed by a live demonstration on a pregnant volunteer. Residents completed pre/post assessments consisting of 12 multiple-choice knowledge questions and 5-point Likert scale items on confidence and interest. Surveys were de-identified for analysis. A paired t-test evaluated knowledge score changes; Cohen’s d estimated effect size. The study was reviewed and deemed exempt by the Institutional Review Board.

Results: Mean pre-test score was 7.0/12 (SD = 1.41) and post-test score was 8.06/12 (SD = 1.80), showing significant improvement (t(17)=2.26, p=0.037); Cohen’s d=0.53. Confidence in ultrasound performance increased (2.18 to 2.29; +5.4%), and interest remained high (4.47 to 4.53; +1.3%). All residents agreed ultrasound education should be integrated into the curriculum.

Conclusions/Implications: A brief, structured session improved resident knowledge and was well received. These results support the feasibility and value of simulation-based ultrasound education and provide a foundation for longitudinal implementation.
