Improving Inpatient Continuous Glucose Monitor (CGM) Data Documentation: A Quality Improvement Initiative

Abstract

Introduction and Objective: Continuous glucose monitoring (CGM) is increasingly used in hospitalized patients to reduce point-of-care testing and improve patient comfort. However, inconsistent documentation of CGM values within the electronic medical record can limit efficient review of glycemic trends and raise patient safety concerns. We evaluated whether implementing a targeted nursing practice alert improved documentation of patient-reported CGM glucose values in the inpatient setting, thereby enhancing glycemic monitoring and clinical decision-making. Methods: We conducted a retrospective pre-post quality improvement (QI) study at Corewell Health William Beaumont University Hospital. Adults ≥18 years with type 1 or type 2 diabetes (DM) admitted with a functional CGM were included. Pre-intervention data were collected from 7/21/24-11/21/24 and compared with a 5-month post-intervention period from 11/21/24-04/21/25 following implementation of the nursing practice alert. Documentation rates were compared using the Chi-square test, and continuous outcomes were analyzed using the Mann-Whitney U test. Results: A total of 224 patients were included (34% type 1, 66% type 2 diabetes); 9 with unknown diabetes status were excluded. Mean age was 63±15 years and median HbA1c 7.4% (IQR 6.4-8.4). CGM documentation increased from 54.3% pre-intervention (n=94) to 68.8% post-intervention (n=128) (χ² p=0.0389), with an average of 3 CGM readings documented daily. Severe hypoglycemia (< 54 mg/dL) decreased from 0.47% to 0.19%. Improved documentation was associated with longer length of stay and older age. Documentation gaps varied by hospital unit. Conclusion: Implementation of a nursing practice alert improved daily CGM documentation and reduced severe hypoglycemia, supporting system-level strategies to improve inpatient glycemic management.
