Outcomes for Atrial Tachy-Arrhythmia Ablation in Patients With ATTR Cardiac Amyloidosis: A Meta-Analysis

Abstract

Background: Atrial Arrhythmias are present in up to 70% of patients at diagnosis of cardiac amyloidosis (CA). The most common subset of CA is transthyretin CA (ATTR-CM). Despite catheter ablation being widely accepted as a rhythm control strategy for atrial arrhythmias in patients without CA, few studies have been conducted in patients with CA. Objective: We aim to analyze all-cause mortality and heart failure hospitalization of studies comparing catheter ablation and non-ablation for treatment of atrial arrhythmias in patients with ATTR-CM. Methods: A systematic search was conducted on Pubmed for studies published between 2010-2025 that assess outcomes of ablation of atrial tachyarrhythmias in patients with ATTR-CM. A total of 305 studies were found. Four studies were selected based on the presence of an experimental (Ablation group) and a control group (Non-Ablation). Statistical Analysis was done via R studio and RevMan 5.18. Results: We evaluated a total of four studies, with a total of 616 patients. The mean age was 75.3 years, and 79.5% were male. 95.1% of patients had Afib, 8.1% had Aflutter, and 13.6% had atrial tachycardia. There was a significant difference between the rates of CHF in the ablation and non-ablation group (62.6% vs 79.9%, P,0.001). Patients that underwent ablation were found to have 74% lower all cause mortality compared to the group non-ablation (OR 0.26, 95% CI 0.16-0.42, P , 0.001) (Figure 1a). Patients who underwent ablation were also found to have less HF hospitalizations during the follow up time (OR 0.13, CI 0.09-0.19, P, 0.001) (Figure 1b). Conclusion: Patients with ATTR-CM submitted to ablation for atrial tachyarrhythmias had lower all-cause Mortality and Heart Failure hospitalization when compared to patients that were not submitted to ablation. This might suggest that catheter ablation is a viable option in treating atrial arrhythmias for patients with ATTR-CM.
