Abstract

Spinal cord ischemia (SCI) remains a devastating complication of thoracic endovascular aortic repair (TEVAR) and thoracoabdominal branch endoprosthesis (TAMBE), with reported incidence rates of 4-18%. We present a 70-year-old woman who developed irreversible SCI following staged TEVAR and TAMBE. Despite aggressive implementation of a multimodal SCI rescue protocol—including cerebrospinal fluid drainage, mean arterial pressure optimization above 100 mmHg, and hemoglobin above 10 m/dL—MRI confirmed spinal cord infarction from T10 through the conus medullaris with permanent flaccid paralysis. This case highlights cumulative SCI risk factors and the importance of being prepared to implement the SCI protocol.
