Abstract
We present a 67-year-old female with sickle cell disease, acute-on-chronic diastolic heart failure, CKD-5, obesity (BMI 39), and RSV undergoing AV fistula creation for dialysis access. Bronchospasm occurred upon induction and airway manipulation, with oxygen saturations dropping to the low 90s in the setting of existing pulmonary edema. RSV-associated perioperative bronchospasm is less commonly described, particularly in patients with significant cardiopulmonary comorbidities. RSV-triggered airway hyperreactivity and increased pulmonary vascular resistance and right heart strain created significant anesthetic challenges. This case reviews why patients develop bronchospasm, what triggers it, and its varied presentations.
