Abstract
A 32-year-old female presented to our Level I Trauma Center with multiple self-inflicted chest stab wounds requiring emergent bedside thoracotomy, which revealed pulmonary artery laceration and significant intrathoracic hemorrhage. Massive Transfusion Protocol (MTP) was activated; however, blood products required retrieval from a blood bank on a different side of the hospital, resulting in an approximately 7-minute delay during active exsanguination. Despite aggressive resuscitation, sustained return of circulation was not achieved. We will discuss the critical importance of minimizing time-to-transfusion during MTP activation, optimizing blood product accessibility in trauma resuscitation areas, and improving MTP efficiency through systems-level coordination.
