Takayasu Arteritis Presenting as Critical Left Main Coronary Artery Disease in a Young Patient

Abstract
A 19-year-old female with asthma presented with chest pain and dyspnea and was found to have critical left main coronary artery stenosis with imaging suggestive of an inflammatory process. Despite medical and mechanical support, she developed ongoing ischemia requiring emergent coronary artery bypass grafting. Intraoperative findings included a thickened, inflamed aorta and fragile coronary vessels. Aortic biopsy demonstrated chronic inflammation, and the overall clinical presentation and imaging findings were most consistent with Takayasu arteritis. Postoperatively, she recovered with preserved cardiac function on immunosuppressive therapy. This case highlights a rare etiology of coronary disease and the importance of multidisciplinary perioperative management.
