Postpartum Hypoxia in Preeclampsia: A Diagnostic and Management Challenge After Cesarean Delivery

Abstract
A 24 year-old G1P0 at 37w2d who was diagnosed with preeclampsia and developed acute hypoxic respiratory distress after c-section complicated by postpartum hemorrhage. Red blood cells and fresh frozen plasma were provided to stabilize the patient throughout her perioperative course. However, on postoperative day 1, the patient developed acute hypoxia requiring further medical evaluation, imaging, medical treatment, and escalated respiratory support. Differential diagnosis included several cardiogenic and pulmonary ideologies--these included TRALI, infection, pulmonary edema related to preeclampsia, and pulmonary embolism. This case will discuss the differential diagnosis and management of acute hypoxia in a preeclamptic parturient in the setting of postpartum hemorrhage.
