Abstract
A 21-year-old female with asthma presented with severe shortness of breath after several days of upper respiratory symptoms. Despite multiple inhaler uses, steroids, bronchodilators, and BiPAP, she developed worsening hypercarbia and acidosis, requiring intubation. Post-intubation, she remained difficult to ventilate with peak pressures in the 50s and persistent respiratory failure. Due to refractory status asthmaticus and worsening acidosis, she was placed on venovenous ECMO via right IJ and right femoral vein cannulation. She later stabilized and was successfully decannulated.vvvv
