Abstract

A 36-year-old female scheduled for elective hysterectomy aspirated during induction, leading to procedure cancellation. She had followed standard NPO guidelines, but later disclosed cannabis use the night before to manage anxiety. This case highlights how cannabis, though increasingly common, may impair gastric emptying and airway reflexes, posing unrecognized perioperative risks. It underscores the importance of thorough preoperative screening, patient education, and updated anesthetic considerations in the context of cannabis use—even when patients appear compliant with fasting protocols.
