Mitral Paravalvular Leak Closure Via Left Anterolateral Thoracotomy

Abstract
An 81-year-old female with a history of rheumatic heart disease and prior mitral and aortic valve replacements presented with fatigue and exertional dyspnea. TEE revealed severe paravalvular leak (PVL). Attempts to advance occluder devices via a transseptal approach were unsuccessful, leading to worsening PVL and reduced LVEF. Given her age and comorbidities, she was not a candidate for redo valve surgery. PVL closure was performed using a combined transseptal and transapical approach, facilitated by a small anterolateral thoracotomy. This discussion will outline the risks and anesthetic considerations associated with this complex approach.
